Paperwork

All students in athletics must have the following papers completed and turned in to the coaches by the first day of school. Physical, Medical History, Rules Acknowledgement, FMS Contract, Information Card, Code of Conduct, Steroid Use form, and Emergency Card.

The papers must be turned in as one packet and all forms must be completed and signed when turned in. Packets may be turned in now.

 
Uniforms

Orders for uniforms may be placed at any time now. The orders will not be filled until next fall.


Please make sure your son or daughter is present to try on the uniform. Once you purchase the uniform, initials will be placed on the uniform or in the tag. You will not be able to return a uniform once it has been marked.

You will also be responsible for getting the last name on the back of the shirt and sweatshirt.
