Please remember that Elementary students CAN NOT transport medications to and from school. All parents MUST pick up and drop off medications. Thank you for keeping our students safe.

CHANGES
New Immunizations for 2009-2010 Kindergarten entry are two of each of the following immunizations:
Hepatitis A, MMR and Varicella. All other students will be phased in.

-State Mandated Screenings for PPCD, Kindergarten, First, Third and Fifth are conducted September thru December each year. For other important information on screenings, click here.

HB 1688, "Allows students to self administer asthma mediations while at school or school functions with permission from parents and physicians."  A school Medication Self-Carry Agreement Form must be completed by the parent and the physician and kept on file with the school nurse.  Principal approval is required and must be updated each school year.  Please complete a Medication Self-Carry Agreement Form and send it to school with your child.  To get a copy of this Form, click here.

 


Hello, my name is Sandra Elizalde, R.N. or “Nurse Sandra”, as many of our students will say.  I am the school nurse for the children and staff of Parkway Elementary. Parkway is a great school and I love serving the students and staff.  The role of the school nurse is very complex and I hope that this web site will offer some understanding of Lewisville's Health Services and the services it offers.
